What to Bring To Your Appointment

New Patients
New patients should print and complete the New Patient Information Packet (link to form) before visiting our office for your first appointment. This packet contains information about appointments, insurance, directions to the office, and the forms you need to complete for our staff.

Appointments
Please plan to arrive 30 minutes before your appointment time to accommodate traffic problems and completion of paperwork. If you need to change or reschedule your appointment, it is very important that you call our office at least 48 hours in advance. Our waiting times can be lengthy, and your notice allows us to schedule other patients for that time.

Your first appointment will involve a comprehensive evaluation of your child's neurological condition with a complete history and physical exam. This visit will last about 45 minutes. Follow-up appointments will last 15-20 minutes, depending on the complexity of the evaluation.

Insurance and Payment
Please bring your most current insurance cards to every appointment. If your insurance plan requires a referral, you must obtain this referral from your primary care physician/pediatrician before your appointment. We will file insurance claims for you, however copays, coinsurance, and deductibles are due in full at the time of service.  Additionally, if our services are not covered by your insurance, payment in full is expected at the time of the visit. For your convenience, we accept MasterCard, Visa, Discover, personal checks and cash.

Please note that even though we are providers for the below listed insurance plans, your particular plan may consider our charges or your child's diagnosis as non-allowable. If any services or diagnoses are considered being non-allowable, the guarantor is responsible for those charges. Additionally, we do not accept commercial insurance at the Marcus Institute location except for EEG and Baclofen appointments.

The following plans do not require a referral, but an in-network physician must refer the patient in order for the visit to be considered in-network by your insurance plan.   The following plans require a referral prior to your appointment. If a referral is not presented to our office at the time of service, you will be responsible for payment in full.

What to Bring To Your Appointment

  • Completed Patient Demographic Information and History forms (download form)
  • Referral authorization number or paper referral (if necessary)
  • Current insurance card(s)
  • MRI or CT films/results (if done within past year)
  • EEG tracing/report (if not performed at our office or Children's Healthcare of Atlanta)
  • Any pertinent medical records from other specialists or primary care physician
  • Check, cash, MasterCard, Visa or Discover for copay, coinsurance, deductibles (if necessary)
  • The maximum parking garage rate is $5.00 (Peachtree Dunwoody location only)
